IMUNOTERAPIA COM ONCO-BCG NO TRATAMENTO DE NEOPLASIA MALIGNA VESICAL

Resumo

RESUMO: A imunoterapia com BCG intravesical é a opção terapêutica eficaz para reduzir o risco de recidiva e recorrência em pacientes com câncer de bexiga não invasivo. O desenvolvimento de uma estirpe de Micobacterium bovis atenuada, denominada Bacilo de Calmete-Guérin / BCG, veio revolucionar o mundo da imunoterapia. Rapidamente o BCG se tornou uma arma terapêutica com um lugar muito próprio no tratamento do carcinoma da bexiga, associando-se a uma diminuição da recorrência, e possibilidade de progressão, após a resseção transuretral da neoplasia vesical, demonstrando taxas de resposta completa de 55-65%. O mecanismo de ação ainda não se encontra totalmente esclarecido, no entanto, muito já se descortinou sobre o modo como o BCG atua no carcinoma da bexiga. Sabe-se que a instilação de BCG, de modo subcutâneo ou intravesical, desencadeia uma cascata imunológica, com ativação do sistema reticulo endotelial. O bacilo é absorvido pelas células uroteliais tanto neoplásicas como saudáveis, sabendo- se que as células pouco diferenciadas captam o bacilo com maior intensidade e são mais sensíveis à resposta por ele desencadeada. Objetivos: Contribuir com o conhecimento sobre os benefícios da imunoterapia com BCG como adjuvante no tratamento de câncer superficial de bexiga em estágio inicial, disponibilizados em hospitais para tratamento oncológicos em Maceió-AL. Trata-se de uma revisão de literatura, utilizadas em bases de dados eletrônicas como Scielo, site do Ministério da Saúde, INCA.  Resultados: A maioria dos estudos aponta para maior eficácia do imuno BCG em comparação com a maioria das nove quimioterapias utilizadas. Uma meta-análise de sete estudos concluiu que a recidiva do tumor foi reduzida com significância por imuno BCG. Além disso, o imuno BCG é o único agente que, além de reduzir o risco de recidiva, impede a progressão da doença. Conclusão: De acordo com a revisão realizada, a utilização de BCG veio revolucionar o tratamento dos carcinomas da bexiga, ocupando agora um lugar de destaque nas opções terapêuticas preservadoras de órgão. O BCG intravesical se mostrou eficaz para reduzir o risco de recidiva e recorrência em pacientes com câncer de bexiga. Seu uso é indicado para tratamento adjuvante em tumores com alto risco de recidiva e progressão, além de tratamento primário para tumores. A instilação intravesical é o procedimento padronizado para administração do BCG, ainda assim, estudos são necessários para compreender quais os tumores que beneficiam efetivamente mais da imunoterapia e o porquê da melhor resposta.

ABSTRACT:

Intravesical BCG immunotherapy is the effective therapeutic option to reduce the risk of relapse and recurrence in patients with non-invasive bladder cancer. The development of an attenuated Micobacterium bovis strain called Calmete-Guérin / BCG Bacillus (in honor of its producers) has revolutionized the world of immunotherapy. Developed in the early twentieth century as a weapon against Tuberculosis, BCG would only later make its foray into the world of Urology in the form of intravesical instillations, following the first results published by Morales in 1977. BCG quickly became a weapon and a possibility of progression after transurethral resection of the bladder neoplasm, demonstrating a complete response rate of 55-65%. The mechanism of action has not yet been fully elucidated, however, much has already been discovered about how BCG works in bladder carcinoma. It is known that instillation of BCG, either subcutaneously or intravesically, triggers an immunological cascade, with activation of the endothelial reticulum system. The bacillus is absorbed by both neoplastic and healthy urothelial cells, it being understood that poorly differentiated cells capture the bacillus with greater intensity and are more sensitive to the response it triggers. Objectives: To contribute to the knowledge about the benefits of BCG immunotherapy as an adjuvant in the treatment of superficial bladder cancer, available in hospitals for cancer treatment in Maceió-AL. This is a literature review, used in electronic databases such as Scielo, a website of the Ministry of Health, INCA. Results: Most studies point to greater efficacy of the BCG immunoassay compared to most of the nine chemotherapies used. A meta-analysis of seven studies concluded that tumor recurrence was significantly reduced by immuno BCG. In addition, the BCG immunologist is the only agent that, in addition to reducing the risk of relapse, prevents the progression of the disease. Conclusion: According to the review, the use of BCG has revolutionized the treatment of bladder carcinomas, now occupying a prominent place in the organ-preserving therapeutic options. Intravesical BCG has been shown to be effective in reducing the risk of relapse and recurrence in patients with bladder cancer. Its use is indicated for adjuvant treatment in tumors with high risk of relapse and progression, besides primary treatment for tumors. Intravesical instillation is the standard procedure for BCG administration, yet studies are needed to understand which tumors actually benefit most from immunotherapy and why the best response.
