Acomete indivíduos de todas as raças e ambos os sexos, com prevalência no sexo feminino, principalmente, mulheres em idade fértil e com fototipos mais altos (especialmente IV-V) e que vivem em áreas com elevada radiação ultravioleta(UV). Sua etiopatogenia, embora inconclusiva, apresenta fatores de riscos conhecidos, como: predisposição genética, radiação solar, gravidez, cosméticos específicos, doenças endocrinológicas e medicamentos, principalmente a terapia de reposição com estrogênio-progesterona e o uso de anticoncepcionais hormonais, além da recém descoberta interação vascularização e melanogênese. Apesar desses fatores, uma grande porcentagem dos acometidos, dentre homens e mais prevalente em mulheres, apresenta caráter idiopático. Diante disso, da sua alta incidência e do curso recorrente e refratário da doença, apresenta bastante dificuldade em seu tratamento. O mesmo tem como principal objetivo o clareamento das lesões, prevenção e redução da área afetada, envolvendo o menor número possível de efeitos adversos. Engloba, atualmente, diversas alternativas isoladas e complementares, como: uso de protetor solar de amplo espectro, cremes de despigmentação( Hidroquinona associada ou não à Tretinoína), Peelings químicos (ácido glicólico e solução Jessner, os responsáveis pelos resultados mais positivos), o afastamento do uso de anticoncepcionais orais e tratamentos de reposição hormonal, dentre outros.<strong> OBJETIVOS: </strong>O presente estudo busca, através de uma revisão de literatura, fazer um levantamento de quais intervenções terapêuticas são mais eficazes para o melasma. <strong>MÉTODOS: </strong>Os artigos utilizados para a elaboração da presente revisão foram colhidos nas plataformas Scielo, PUBMED e LILACS, sem restrição de idioma. <strong>CONCLUSÃO: </strong>Observa-se através dessa revisão de literatura que até o momento não há terapia com resultados totalmente satisfatórios. No entanto, a Hidroquinona (HQ) permanece como agente isolado de maior eficácia, mostrando-se segura e com poucos efeitos adversos, tanto no tratamento como no preparo da pele para receber peelings químicos ou físicos. A HQ e o Ác. Retinóico têm seus efeitos potencializados quando combinados à corticosteroides. Diante disso, pode-se concluir que os pilares do tratamento do melasma se dá pela combinação de Hidroquinonas, Tretinoína e um corticosteroide, associada à educação dos pacientes com relação ao uso regular de filtro solar e a exposição solar.</p><p> </p><p>Palavras-chave: Hidroquinona. Tratamento.</p><p> </p><p> </p><p><strong>ABSTRACT: INTRODUCTION</strong>: Melasma is an acquired chronic hypermelanosis, it is characterized by hyperchromic macules at skin exposure sites, such as the frontal and malar regions. It affects individuals of all races and both genders, being predominant in females, especially in women of childbearing age and with higher phototypes (especially IV-V) and living in areas with high incidence of ultraviolet (UV) radiation. Its etiopathogenesis, although studies are inconclusive, presents known risk factors, such as: genetic predisposition, solar radiation, pregnancy, specific cosmetics, endocrinological diseases and medications, mainly estrogen-progesterone replacement therapy and the use of hormonal contraceptives, as well as the newly discovered interaction vascularization and melanogenesis. Despite these factors, a large percentage of those affected, including men but more prevalent in women, presented idiopathic character. Therefore, due to its high incidence and the recurrent and refractory course of the disease, it results in a great difficulty in its treatment. The main objective is bleaching the lesions, prevention, and reduction of the affected area, involving the fewest possible adverse effects. It currently includes several isolated and complementary alternatives, such as: the use of broad spectrum sunscreen, depigmentation creams (Hydroquinone associated or not with Tretinoin), chemical peels (glycolic acid and Jessner solution, those responsible for the most positive results), the removal of oral contraceptive use and hormone replacement therapy, among others. <strong>OBJECTIVES</strong>: The present study means to make, through a literature review, a survey of which therapeutic interventions are most effective for melasma. <strong>METHODS</strong>: The articles used in the making of the present review were collected on the Scielo, PUBMED and LILACS platforms, without language restriction. <strong>CONCLUSION</strong>: It was observed, through the literature review, that, until now, there is no therapy with fully satisfactory results. However, Hydroquinone (HQ) remains the isolated agent of greater efficacy, being safe and have only a few adverse effects, both in the treatment and preparation of the skin to receive chemical or physical peels. The HQ and retinoic acid have greater effects when combined with corticosteroids. Therefore, it can be concluded that the pillars of the treatment of melasma is an combination of Hydroquinones, Tretinoin and a corticosteroid, associated with the education of the patients regarding the regular use of sunscreen and sun exposure.</p><p> </p><p> </p><p><strong>Keywords:</strong> Hydroquinone.
